There's a burning sensation in your mouth even though you haven't had anything hot to eat or drink. It's an experience you've had for years, often accompanied by mouth dryness, tingling or numbness that leaves you irritable, anxious or depressed.
The root causes for Burning Mouth Syndrome (BMS) remain elusive, although there appear to be links to diabetes, acid reflux, menopausal hormonal changes or even psychological issues. Although we may not be able to pinpoint the root cause we can identify contributing factors to BMS through a detailed oral examination and medical history (including drugs you're taking).
Mouth dryness is one of the most common factors for BMS. The lack of lubrication from adequate saliva flow can contribute substantially to the irritating burning sensation. There are a number of causes for mouth dryness, including as a side effect from many medications or other treatments.
We must also consider whether an allergic reaction — the body's over-reaction to a foreign substance — may have a role in your symptoms. Some people react to sodium lauryl sulfate, a foaming agent found in many types of toothpaste, along with whitening substances or flavorings like cinnamon; denture wearers can become allergic to the plastic materials used to construct the denture. These, as well as spicy foods, smoking or alcohol, can irritate or cause the tissues lining the inside of the mouth to peel.
Determining what factors contribute to your symptoms allows us to develop a treatment approach tailored to your situation. If, for example, we've determined your BMS stems from dry mouth as a side effect to medication, we can ask your doctor to prescribe an alternative, increase your water intake when taking pills or stimulate saliva flow. If we identify an allergen as a factor, you can eliminate the substance to reduce symptoms.
You may also need to make changes to your eating and lifestyle habits: stop smoking, reduce your alcohol or coffee consumption and avoid very hot or spicy foods. And look for ways to reduce stress, another contributing factor, through relaxation techniques, exercise or support groups.
It's possible that BMS will resolve itself over time. In the meantime, though, we can help you find ways to alleviate the irritation.

Bacteria are bad… right? They can cause diseases like pneumonia, strep throat, and tooth decay. They are the reason we wash our hands with soap (or antibacterial gels) and cook (or refrigerate) our food. Yet it turns out that bacteria are also necessary to keep our bodies healthy — and new research is showing just how important these tiny microorganisms are to our well-being. Here are five facts you should know about bacteria.
The bacteria in our bodies outnumber our cells by a factor of 10 to 1. An estimated 100 trillion bacteria live inside the average human — but because they’re so small, they make up only 1-3 percent of our body mass.
The collection of bacteria we harbor is called our “microbiome.” Like the groundbreaking study of human DNA called the Human Genome Project, recent research is leading to a “map” of our bacterial makeup. This revolutionary study is called — you guessed it — the Human Microbiome Project.
No two people have exactly the same microbiome. But in general, the bacteria that live in a particular spot on the body (the mouth, for example) play the same roles in different individuals. Research has also shown that a healthy microbiome looks very different from a diseased microbiome.
In terms of bacteria, the mouth is one of the best-understood areas of the body. It has long been known that tooth decay can result when “bad” oral bacteria begin to outnumber their “good” counterparts. Now we are gaining a better understanding of how certain lifestyle factors — like cigarette smoking — may influence the bacterial balance in the mouth.
Understanding the microbiome may lead to new treatments for disease. Researchers hope that one day, certain serious diseases could be controlled by bacterial “transplants” that re-balance an individual’s microbiome. Maintaining a healthy microbiome could also help prevent many diseases.
So by all means, don’t stop brushing your teeth or washing your hands — this helps control bacteria that could harm you — but do remember that not all bacteria are harmful. One day, an infusion of bacteria might just cure your illness.
Most often, all of your child’s primary teeth will eventually be replaced by permanent teeth, but you shouldn’t consider them less important — there are serious consequences for losing a primary tooth prematurely. Besides providing a means for a child to chew food and speak clearly, primary teeth also save space for the permanent teeth to erupt; a premature loss could lead to malocclusions (bad bites) that may result in costly orthodontic treatment later.
That’s why it’s important to fight tooth decay in primary teeth. By keeping them healthy and in place until it’s time for their departure, their permanent replacements have a better chance of erupting into their proper positions.
Here are 4 tips for preventing tooth decay in primary teeth:
Begin daily oral hygiene when teeth first appear. Begin brushing with fluoride toothpaste as soon as the first primary teeth come in. Brushing removes bacterial plaque, the primary cause of tooth decay, and fluoride strengthens enamel. Because they tend to swallow toothpaste rather than spit it out, use just a smear of toothpaste for infants and toddlers, and a pea-sized amount for ages two and older.
Start regular dental visits by the child’s first birthday. By beginning regular checkups around age 1, we’ll have a better chance of discovering developing tooth decay or other problems early. You’re also setting a good foundation for what should be a lifelong habit for optimum dental health.
Limit sugar consumption. The oral bacteria that cause tooth decay feed on leftover carbohydrates like sugar, so you should limit intake especially between meals. One culprit to watch out for: a bedtime bottle filled with formula, milk or fruit juices, all of which contain carbohydrates (sugar). Water or no bottle at all is a better alternative.
Consider topical fluoride or sealants for extra protection. In some circumstances, we may advise protecting the enamel of newly erupted teeth with an applied sealant. These protective coatings fill in porous pits and fissures in young teeth to deny access to disease. Supplemental fluoride will further strengthen young tooth enamel.
Taking these measures and remaining vigilant to the first signs of decay can go a long way toward preserving your child’s teeth. Their future oral health depends on it.

Like most people, you’ve no doubt experienced occasional dry mouth as when you’re thirsty or just waking from sleep. These are normal occurrences that usually don’t last long.
But xerostomia or chronic dry mouth is another matter. Not only is this continual lack of adequate saliva uncomfortable, it could increase your risk for tooth decay or periodontal (gum) disease.
What’s more, chronic dry mouth can have a number of causes. Here are 3 common causes and what you can do about them.
Inadequate fluid intake. While this may seem obvious, it’s still common—you’re simply not consuming enough water. This deprives the salivary glands of adequate fluid to produce the necessary amount of saliva. If you’re regularly thirsty, you’ll need to increase the amount of water you drink during the day.
Medications. More than 500 drugs, both over-the-counter and prescription, can cause dry mouth as a side effect. This is one reason why older adults, who on average take more medications, have increased problems with dry mouth. There are some things you can do: first, talk with your healthcare provider about alternative drugs for your condition that are less likely to cause dry mouth; drink more water right before taking your medication and right afterward; and increase your daily intake of water.
Diseases and treatments. Some systemic diseases like diabetes or Parkinson’s disease can lead to xerostomia. Autoimmune conditions are especially problematic because the body may turn on its own tissues, the salivary glands being a common target. Radiation or chemotherapy treatments can also damage the glands and lead to decreased saliva production. If you have such a condition, talk with your healthcare provider about ways to protect your salivary glands.
You can also ease dry mouth symptoms with saliva boosters like xylitol gum or medications that stimulate saliva production. Limit your intake of caffeinated drinks and sugary or acidic foods. And be sure you stay diligent with your oral hygiene habits and regular dental visits to further reduce your risks of dental disease.

They seemingly pop up out of the blue inside your mouth: tiny sores that are sometimes painful — and always annoying. Then, in about a week to ten days these small, irritating lesions are gone.
They're known as canker sores: the most common break out in the linings of the mouth, including the cheeks, lips, under the tongue or even the back of the throat. Medically known as aphthous ulcers, you'll recognize these round lesions by their yellow-gray center surrounded by a red “halo.”
You might feel a tingling sensation a couple of days before an outbreak. Once they appear they usually last a week to ten days; during that time they can cause discomfort especially while eating or drinking.
We don't know fully what causes canker sores, but it's believed they're related to abnormalities in the immune system, the processes in the body that fight infection and disease. High stress or anxiety and certain acidic or spicy foods like citrus fruit or tomato sauce also seem to trigger them.
Most people experience canker sores that range in intensity from slight discomfort to sometimes severe pain. But about 20-25% of people, mostly women, have an acute form known as recurrent aphthous stomatitis (RAS). Thought to be hereditary, RAS produces clusters of ulcers that are almost always painful, and which come and go on a regular basis.
Our main treatment goal with canker sores is to decrease discomfort while the outbreak runs its course and promote rapid healing. There are over-the-counter ointments that often prove effective. For more resistant symptoms we can also prescribe topical or injectable steroids or other medications.
Canker sores are rarely concerning as a significant health issue. You should, however, take an outbreak seriously if it hasn't healed within two weeks, if the outbreaks seem to be increasing in frequency or severity, or you're never without a sore in your mouth. In these cases, we may need to take a tissue sample of the lesion to biopsy for signs of cancer, pre-cancer or some other skin disease.
More than likely, though, the canker sore will be benign albeit annoying. With effective treatment, though, you can get through the outbreak with only a minimal amount of discomfort.
